Min Qualifications

High School Diploma or equivalent and one year of relevant demonstrated community organization or outreach experience. Must obtain Community Health Worker Certification from the Texas Department of State Health Services (TX DSHS) within the first twelve (12) months of employment, which must be kept current in accordance with TX DSHS standards.

Job Description:

Responsible for health education and outreach activities in the community, including organizing and leading fitness and nutrition activities and building rapport with diverse populations and local partner agencies. Maintains professional relationships with community organizations and health clinics and works with community health educators to meet the health needs for the area.

Salary Range

From $19,040 to $28,560 annually

This position is a Band Code: CS11

Specific Job Related Duties

CHW will serve as liaisons between the clinic and the community to facilitate access to the health and/or social services the patients require. These resources may include health insurance, food, housing, transportation, clothing and other health related services. The CHW, with the nurse manager, will help patients understand their health conditions and work with them to develop strategies to improve their health and well-being. The CHW will educate and inform patients of the RMCHP services and grants. The CHW will do home visits when necessary.

Must be able to travel to other UTMB clinics and do home visits as needed.

Preferred Language

Seeking an individual with strong Spanish and English speaking skills.

Preferred License/Certificate

Must have Community Health Worker Certification from the Texas Department of State Health Services (TX DSHS).
